Job summary

Alliance Health & Human Services in Marlborough, MA is seeking an Activities Assistant to support daily recreation programs for residents. You will help plan and implement activities, engage residents in goal-directed plans, and assist with individualized care plans under the supervision of the Activity Director.

The role requires a high school diploma, friendly demeanor, teamwork, and the ability to transport residents to program activities.

Responsibilities

• Assist in planning and implementing a recreation program.

• Transport residents to and from recreation programs.

Job Qualifications:

• Friendly, outgoing personality, with a warm, caring and patient attitude.
• Willingness to work as part of a team.

Essential Job Functions:

• Assists in planning and implementation of organized program for recreation
• Provides a planned resident specific, goal directed recreation program for assigned residents/patients; facilitates small group activities as assigned by the director
• Ensure that all charted recreation progress notes are informative and descriptive of the services provided and indicate the residents/patient’s response to the service. Assists in developing a written comprehensive plan of care for each resident/patient recreation program that identifies the problems/needs of the resident/patient and the goals to be accomplished for each problem/need. Follow the established plan of care in providing the daily recreation program.
• Involve the resident/patient and family members in the assessment and care planning process when possible.
• Transport residents/patients to and from recreation programs.
