Description
The Activity Assistant is a vital part of our direct care team, supporting older adults with Alzheimer’s, dementia, and related cognitive impairments through meaningful engagement and direct care. This role leads group and individual activities, implements the monthly activity calendar, and helps maintain a safe, welcoming space. In addition to facilitating person-centered programming, the Activity Assistant provides hands-on support with daily needs, such as mobility assistance, prompting, and redirection. They work closely with participants and caregivers, offering compassionate service while modeling dignity, empathy, and professionalism for volunteers and staff.
Responsibilities
- Lead and actively engage participants in group activities, such as exercise, current events, discussion groups, art & music classes, gardening, and table games. Ensure the successful implementation of the activity program by creating a dynamic, inclusive, and engaging environment that aligns with Person-Centered Care (PCC) principles.
- Encourage socialization among participants by creating opportunities for meaningful connections and conversations, supporting positive peer interactions, and reducing feelings of isolation.
- Adapt activities creatively to meet the varying interests, abilities, and moods of participants.
- Provide direct support including assistance with toileting, transfers, mobility, and medication reminders, while respecting dietary restrictions and allergies for safety and well-being.
- Welcome participants each morning with a friendly greeting to support a smooth transition, and offer thoughtful farewells at departure to foster connection.
- Demonstrate proficiency in incontinence care to maintain participants' dignity and hygiene.
- Collaborate with the activities coordinator to manage complex care needs, ensuring appropriate support and resources.
- Model excellent customer service and professionalism.
- Perform opening and closing tasks to ensure smooth program operation.
- Record participants' progress and observations accurately in the Electronic Health Record (EHR) system daily.
- Proactively address challenges and improve participant experiences and team operations.
- Communicate effectively with supervisors about notable behaviors or issues.
- Prioritize tasks efficiently, balancing urgent needs with program quality.
- Set a high standard for volunteers by demonstrating professionalism and leadership.
- Adhere to the Standards of Conduct outlined in the Yolo Hospice, Inc. Integrity Program.

Requirements
- At least one year of experience in a social, recreation, or educational program within the past five years, including one year full-time, and experiential knowledge of elderly or disabled adult needs.
- Ability to meet background screening requirements.
- Current CPR certification or willingness to obtain one within 90 days of employment.
- Excellent written and verbal communication skills.
- Ability to work independently with minimal supervision.
- Understanding of the social, mental, and physical needs of elderly individuals related to recreation.
- Ability to perform under stress and handle unexpected events.
- Ability to perform repetitive, detailed tasks, stand or walk for 8 hours, and lift up to 50 pounds.
- Valid California driver’s license and insurance if driving for work.
- Flexibility to adjust hours as needed for program requirements.
Salary: $39,520 - $43,680 annually
