Job summary

Yamato Nursing and Rehabilitation Center in Boca Raton, FL is seeking an energetic Activities Assistant to support the Activities Director in planning programs that engage memory care residents and promote social, mental, and physical activity. The role requires coordinating monthly calendars, interviewing residents for preferences, leading group and individual sessions, and documenting participation.

A HS diploma and relevant training are preferred; a memory care background is highly valued.

Qualifications

• Memory Care Unit experience preferred.
• Alzheimer's and Dementia care certification preferred.
• High School Diploma required; recreation coursework a plus.
• Coursework in recreation is preferred.
• Completion of a state-approved activity training course is preferred.
• Experience leading recreational activities including arts & crafts and community outings is desired.

Responsibilities

• Develop a monthly activities calendar (large print) and post it throughout the facility and in each resident room.
• Interview residents/families to identify activity preferences.
• Coordinate and assist residents in traveling to activity locations.
• Lead or support group and one-on-one activity sessions to boost physical, emotional, and social well-being.
• Document attendance and progress notes for each resident's participation.
• Observe and report changes in resident mood or participation to the Activities Director.
• Involve residents and families in planning/executing activity programs when appropriate.
• Assist with organizing offsite trips and coordinating transportation.
• Collaborate with other departments to ensure smooth activity execution.
• Support Resident and Family Council meetings as needed.

Facility: Yamato Nursing and Rehabilitation Center
Location: Boca Raton, FL
Shift: 11:00 AM – 7:30 PM
Salary: $17.00 per hour

Job Summary:

Yamato Nursing and Rehabilitation Center is seeking a compassionate and energetic Activities Assistant to join our team. In this role, you will support the Activities Director in planning and executing programs that promote socialization, mental stimulation, and physical engagement for our residents—especially those in our Memory Care Unit.

Key Responsibilities:

• Assist in developing a monthly activities calendar (in large print) and post it prominently throughout the facility and in each resident’s room.
• Interview residents and/or family members to identify activity preferences.
• Coordinate and assist residents in traveling to activity locations.
• Lead or support group and one-on-one activity sessions designed to enhance residents’ physical, emotional, and social well-being.
• Accurately document attendance and progress notes for each resident's participation.
• Observe and report changes in resident behavior, mood, or participation levels to the Activities Director.
• Involve residents and families in the planning and execution of activity programs when appropriate.
• Assist with organizing offsite trips and coordinating transportation.
• Collaborate with other facility departments to ensure activities are carried out smoothly.
• Support Resident and Family Council meetings as needed.

Qualifications:

• Experience working in a Memory Care Unit preferred.
• Certified in Alzheimer's and Dementia care preferred.
• High School Diploma required; coursework in recreation is a plus.
• Completion of a state-approved activity training course is preferred.
• Previous experience in leading recreational activities, including arts & crafts, educational programs, and community outings, is desired.
