Responsibilities include coordinating activities with staff, assisting with discharge planning, transporting residents for events, and maintaining documentation.

Qualifications

• High School diploma required.
• At least 1 year experience in long-term care.
• Ability to plan, organize and evaluate resident activities.

• Observe attendance, behavior and involvement in programs.
• Encourage resident participation in activities.
• Transport residents to and from activities as needed.
• Assist in discharge planning and activity care plans.
• Interview residents or families to obtain activity information.
• Arrange transportation for outings.
• Coordinate activities with other departments and staff.
• Maintain ongoing quality assurance for the activity department.
• Complete necessary forms and reports.
