Clarvida is seeking an ACT Nurse LPN to join our Assertive Community Treatment (ACT) Team serving adults with severe mental illness. The LPN will provide nursing services in community settings, coordinate with a multidisciplinary team, and support clients toward recovery and independence.

This role involves community-based visits, rotating on-call shifts, use of a company vehicle for client transport, and benefits including paid time off and comprehensive insurance options.

Competenze

• Active LPN license in Louisiana.
• 2) Experience with individuals with mental illness and/or substance use disorders.
• 3) Psychiatric nursing experience preferred.
• 4) Valid driver’s license and automobile insurance.

Mansioni

• Provide nursing services as part of an ACT multidisciplinary team.
• 2) Meet clients in homes and community settings.
• 3) Conduct routine health assessments, including vitals and basic labs.
• 4) Support medication management and long-acting injectable medications.
• 5) Coordinate care with medical and psychiatric providers.
• 6) Support clients with severe mental illness and/or substance use disorders.
• 7) Participate in rotating on-call schedule (~1 week every 7–10 weeks).
• 8) Provide client transportation using a company vehicle when needed.
• 9) Maintain accurate, timely, and compliant documentation.
