Job summary

Columbia Community Mental Health is seeking a Counselor II for the ACT Team to deliver therapeutic services within an integrated care approach. The role focuses on assessments, treatment planning, wraparound case management, and coordination with clinical staff to support adults and families.

Candidates should have a Master’s degree in a related field and experience in behavioral health. Shift is Monday–Friday 8:00am–5:00pm with a comprehensive benefits program for employees and families.

Qualifications

• Master’s degree in psychology, Counseling Psychology, Social Work or related field from an accredited college required.
• Work experience in a behavioral or mental health setting preferred.

Responsibilities

• Assessments/Evaluations/Treatment planning.
• Provide wraparound and/or case management.
• Provide care coordination as needed.
• Individual/group skills training according to client treatment plan.
• Liaison with other human service organizations.
• Documentation and reporting as required.
