Job summary

Carolina Outreach in Jacksonville, NC is seeking a compassionate Psychiatric Nurse (RN) to join our ACT Team, delivering community-based mental health services to individuals with complex needs.

You will oversee medical and clinical treatment, collaborate with an interdisciplinary team, provide medication monitoring and crisis assessments, and help create transition plans as clients move through services. Licensure in NC required.

Qualifications

• RN license to practice in NC.
• A valid driver’s license and current automobile insurance.
• ACTT training for ACT teams required.

Responsibilities

• Monitor and oversee all medical/clinical aspects of services delivered.
• Provide medication monitoring and documentation.
• Deliver psychopharmacologic and medical services within scope of practice.
• Collaborate with ACT team to monitor clinical status and response to treatment.
• Conduct crisis assessments, symptom management, and intervention planning.
• Provide individual counseling and psycho-educational support.
• Assist in creating transition plans when clients move in and out of services.
