Job summary

RHA Health Services is hiring an ACT Nurse Type: Regular to join the Assertive Community Treatment Team. The RN will provide oversight and nursing care to individuals with behavioral health needs in the community, following crisis plans and participating in interdisciplinary teams to deliver comprehensive services.

Requires active RN license; Bachelor's degree preferred and at least one year of nursing experience, with behavioral health nursing experience preferred.

This position may be a member of a team that provides support, services, and treatments necessary to provide integrated crisis response, crisis stabilization interventions, and crisis prevention activities or services related to specialized populations that require intensive interventions. Provides oversight and general nursing care and services to persons with behavioral health challenges in the community by performing the duties listed in this job description. Relies on education, skill, and judgment to provide quality services to the people we serve with limited direction and supervision.

Education, Licensure, and Experience required for the position

Requires a unencumbered Registered Nursing License. For certain behavioral health services additional requirements may apply.

Physical requirements to perform essential functions of the job

• Required to lift 20 lbs.
• Must be able to lift a minimum of 40 lbs.
• Must be able to pull minimum of 25 lbs.
• Must be able to squat, kneel, crawl, crouch, climb, and stoop.
• Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls.
• Required to regularly stand and walk.
• Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting.
• Vision requirements include close vision, distance vision, and peripheral vision.
• Must be able to talk and hear.

RHA's Assertive Community Treatment Team (ACTT) offers a full range of support services to people with serious behavioral health needs. ACTT promotes rehabilitation and independence by teaching people coping skills needed to live in their own communities safely and productively, engaging them in normal daily routines and healthy social interactions. People receiving ACTT services often experience significant reductions in crisis situations such as homelessness, incarceration, or repeated hospitalizations.

Job Responsibilities

• Following the supports and services as indicated in the recipients Crisis Plan, which is a component of all community support plans
• Participating as member of interdisciplinary team, attending community support plan (CSP) meetings, developing plans as requested
• Monitoring consumers and taking appropriate action for side effects and adverse reactions to medications
• Assisting team members with assessing learning needs of the consumers and carries out teaching as indicated

Requirements

• Active RN license.
• Bachelors of Nursing preferred.
• Prefer one year of nursing experience.
• Experience in behavioral health nursing preferred.
• Valid drivers license (this is a condition of employment) including personal vehicle insurance coverage.
• Must maintain proficiency in company sponsored training and certifications and privileging if required.

• Education verification and other credentialing based on position requirements.
