Job summary

Hope Network in Flint, MI is seeking a registered nurse to join an ACT team, delivering integrated mental health care and support to individuals with serious mental illness. This full-time role emphasizes a recovery-centered approach, crisis support, and collaboration with an interdisciplinary team.

The position requires Michigan RN licensure, at least one year of clinical experience, and willingness to be on call 24/7.

Qualifications

• RN license to practice in Michigan.
• 1 year+ experience in clinical or rehabilitation setting with persons with severe mental illness.
• Valid Michigan driver's license.
• Must complete ACT-specific initial training within six months of hire and annual ACT trainings thereafter.

Responsibilities

• Promote a recovery-centered, person-centered environment and connect individuals with community resources.
• Develop and update individualized treatment plans with individuals served and the interdisciplinary team.
• Coordinate and monitor services with regular face-to-face engagement and comprehensive activity tracking.
• Maintain accurate consumer records and documentation per standards.
• Provide crisis intervention and on-call support (nighttime, evenings, weekends).
• Provide substance use treatment and support services as appropriate.
• Manage the medication system including MAR oversight, storage, procurement, destruction, and administration/documentation.
• Support psychotropic medication management including prior authorizations and labs.
• Conduct medical assessments and health screenings and promote wellness and education.
• Use motivational interviewing and recovery-centered approaches to support adherence.
