Job summary

Terros Health in Tempe, AZ is seeking an ACT Housing Specialist to join our interdisciplinary clinical team supporting individuals with SMI or mental health needs under Court Ordered Treatment at our Priest Recovery Health Center. The role focuses on locating affordable housing, coordinating services, and advocating for tenants, with a schedule Tue-Sat, 7am-4pm, and a strong benefits package.

Qualifications

• 3 years experience in delivering behavioral health services.
• Bachelor's in Behavioral Health or related field with 1 year of behavioral health experience.
• Master’s Degree in Behavioral Health.
• 3 years experience as housing specialist preferred.
• Must understand housing resources in Maricopa County.
• Must have a valid Arizona driver’s license and meet Terros Health driving policy.
• Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply within 7 working days of employment.
• Microsoft Word, Excel, PowerPoint, Outlook; NextGen preferred.
• Pass TB test, background check, and pre-employment screenings.
• Must pass a 25 wpm typing test.

Responsibilities

• Protect tenancy rights of clients.
• Develop individual service plans; advocate and liaise with families/community agencies.

Job Details: Job Location: Priest Drive - Tempe, AZ 85281, Position Type: Full Time, Education Level: High School Diploma/GED, Salary Range: $19.12 - $22.87 Hourly, Travel Percentage: Up to 50% , Job Shift: Day Shift, Job Category: Behavioral Health/Social Work , Seeking an ACT Housing Specialist to join our interdisciplinary ACT clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Priest Recovery Health Center in Tempe, AZ.

Additional Language Differential Pay Available.

Schedule: Tues - Sat; 7am - 4pm

Must have very flexible schedule as the team provides treatment 7 days/week, including weekends and holidays

IN ADDITION - YOU RECEIVE $3/hour ACT Differential, plus on-call stipend

Duties include:

• Provides direct treatment, rehabilitative and supportive services that support independent living in a community setting to members.
• Works in conjunction with the patient and ACT team staff to assess patient needs and wants, assist the patient in locating housing of their choice, assists in accessing affordable, safe, decent, and permanent housing.
• Supports patients in protecting their tenancy rights regardless of participation in ACT team services.
• Participates in the development of individual service plans, acts as a patient advocate and liaisons with families and community agencies.
• Provides on-call crisis intervention services, as well as weekend and holiday coverage. (schedule flexibility required)

Qualifications: Qualifications:

• 3 years experience in the delivery of behavioral health, or
• Bachelor's in Behavioral Health or related field with 1 year of behavioral health experience, or
• Master’s Degree in Behavioral Health field
• 3 years working as a housing specialist preferred - minimum of 1 year required
• Must have a good understanding of housing resources within Maricopa County
• Must be 21 years of age or older, with a valid Arizona driver’s license, and meet requirements of Terros Health’s driving policy
• Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for one within 7 working days of employment.
• Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred.
• Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
