Qualifications

Responsibilities

• Coordinate health care needs of assigned consumers in the community.
• Develop and implement individualized health treatment plans.
• Maintain medical records and complete health assessments annually.
• Provide case management and service coordination per ACT program standards.
• Participate in team meetings and contribute to treatment planning.
• Monitor and administer medications and injections as required.
