Job summary

Horizon House, Inc. in Delaware County, PA is seeking a dedicated Registered Nurse for our multidisciplinary Forensic Assertive Community Treatment (FACT) team. The RN will conduct psychiatric assessments, monitor physical health, and coordinate care with psychiatrists and the ITT.

Qualifications

• Board-certified RN licensed in Pennsylvania.
• Licensed Registered Nurse in PA and in good standing.

Responsibilities

• Conduct psychiatric assessments and assess physical health needs.
• Administer and manage medications with the psychiatrist.
• Provide treatment, rehabilitation, and support services within the ACT team.
• Coordinate services and case management for assigned clients.
• Develop, write, implement, and revise treatment goals with clients.
• Educate and support clients’ families and advocate for client rights.
• Participate in comprehensive assessments of psychiatric history, status, and risk.
• Transcribe and chart psychotropic medications; monitor effects and side effects.
• Provide shift supervision and be available for on-call emergencies.
• Must be able to travel locally and possess a valid driver’s license.
