You will provide community-based, in-home therapy and employment support for individuals with mental health and substance use disorders, focusing on promoting symptom stability and improving daily functioning.

Qualifications

• Bachelor's degree in Human Services/ Psychology required.
• Commitment to trauma‑informed, person-centered care.
• Ability to collaborate with a multidisciplinary team.
• Strong organizational and communication skills.
• Must be free from illicit substance use.

Responsibilities

• Provide community-based, in-home therapy and employment support within ACT.
• Promote symptom stability and enhance community functioning.
• Support clients in employment, housing, and education goals.
• Maintain accurate documentation and treatment plans for the PCP.
