Job summary

Qualifications

• MHRT/C (Community) certification required.
• Valid driver’s license and reliable transportation required.
• Experience with individuals living with severe and persistent mental illness preferred.

Responsibilities

• Provide community-based case management to adults with severe mental illness.
• Meet clients in homes and community settings to assess needs and promote recovery.
• Collaborate with psychiatrists, nurses, social workers, counselors, and peer specialists on treatment plans.
• Assist clients with accessing healthcare, housing, employment, benefits, and transportation.
