Job summary

Family Service League is seeking a full-time Case Manager for the Assertive Community Treatment (ACT) program. The Case Manager conducts home visits and provides education to clients. The Case Manager also provides treatment plans and connects clients to treatment facilities.

The role emphasizes home visits, psycho-education, and collaboration with care teams to ensure access to benefits and community resources. NY driver’s license is required.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree preferred.
• At least one year of experience with SPMI population and familiarity with DSS, SSI, Medicaid Managed Care and Housing.
• Excellent organizational and interpersonal skills required.
• Strong communication skills, both verbal and written.
• Ability to multi-task and prioritize in a fast-paced environment.
• Proficient computer skills, including Microsoft Office.
• Valid and clean New York State Driver's License required.

Responsibilities

• Complete Entitlement Applications.
• Accompany clients to Entitlement Offices.
• Provide money management and budgeting skills.
• Ensure active entitlements (Medicaid and/or other benefits).
• Provide psycho-education to families and clients.
• Facilitate groups.
• Complete client assessments using relapse prevention plans.
• Write treatment plans.
• Provide motivational interviewing.
• Connect clients to treatment facilities.
• Correspond with managed care companies.
• Apply for benefits (Medicaid, SSI/SSD, HEAP, Food Stamps, etc.).
• Provide crisis intervention.
• Cover the crisis phone.
• Participate in daily team meetings and case conferences.
• Conduct daily home visits.
• All other duties as assigned.
