Job summary

You will participate in assessments, crisis response, and linkage to community resources, often working non-traditional hours, and using a personal vehicle for client transport.

Qualifications

• Bachelor's degree in Social Work, Psychology or related field required.
• CM-II certification or will need to obtain CM-II certification within 60 days of employment.
• Valid driver’s license with satisfactory driving record; use of personal vehicle for travel required.

Responsibilities

• Maintain working knowledge of ACT principles and fidelity to model.
• Attend daily organizational meetings and weekly treatment team meetings with multi-disciplinary team.
• Provide routine medication delivery and support to clients in the community.
• Promote healthy lifestyles and wellness education for chronic conditions.
• Refer ACT clients to community resources for basic needs and link services as needed.
• Provide psychosocial rehabilitation services to support functioning in the community.
• Assist with ACT initial comprehensive assessments and ongoing clinical assessments.
• Provide assessments for high-risk clients and follow agency crisis protocols.
• Respond to crises in the community and coordinate with law enforcement when needed.
• May work non-traditional hours including evenings, weekends, holidays, or on-call overnight.
• Utilize agency cell phone for ACT work and be responsive during scheduled shifts.
