Responsibilities

• Provide services in the community where the consumer may need them.
• Provide outreach, education, support, referral to services and connection of resources.
• Show assertive outreach to engage consumers.
• Improve outcomes such as reduced hospitalizations and increased housing stability.
• Develop and document person-centered treatment goals with client input.
• Meet documentation standards and productivity expectations.
• Attend clinical supervision and participate in staffing.
• Deliver crisis management and coordinate emergency services when needed.
• Take on-call shifts after hours and weekends.
• Drive to community settings to provide services.
• Maintain the client's clinical schedule in the electronic health record.
