Job summary

Wyandot-Behavioral-Health-Network is seeking an ACT Therapist to provide community-based individual therapy and crisis services for adults with severe mental illness. You will join an ACT multi-disciplinary team and coordinate with other services to prevent hospitalizations and support recovery.

The role requires a Master’s Degree and Kansas QMHP licensure (or ability to obtain LMAC) within 90 days, plus strong interpersonal and communication skills and proficient Microsoft Office use.

Qualifications

• Master’s Degree and current licensure in Kansas as a QMHP and Master Level Addictions Counselor (LMAC). If not licensed as a LMAC, licensure within 90 days of employment is required.
• Prior experience in mental health, homelessness, or crisis services preferred.
• Ability to establish relationships with individuals and organizations within social services, including civic groups and government entities.
• Demonstrated interpersonal and communication skills.
• Proficient in technology, including Microsoft Office.
• Must have dependable transportation, valid driver’s license and auto insurance.
• Must be able to pass background screenings (criminal history and Kansas Child Abuse/Neglect Registry).

Responsibilities

• Provide community-based intensive therapy and other services to adults with severe mental illness, including possible co-occurring substance use disorders.
• Deliver mental health, dual diagnosis and/or substance use disorder therapy in individual and group settings.
• Maintain caseload within ACT team and other teams as needed.
• Participate in diversion planning and team coordination to reduce hospitalizations.
• Attend daily multi-disciplinary team meetings for coordinated care.
• Engage in research, development, and implementation of new multi-system practices.
• Coordinate mobile emergency responses with relevant teams as needed.
• Participate in on-call or after-hours coverage when required.
• Outreach individuals needing mental health services with frequent law enforcement contact.
• Provide transportation accompanying clients to activities or appointments.
• Complete all required documentation thoroughly and on time.
• Attend supervision sessions as required by supervisor.
• Provide crisis prevention and intervention as needed.
• Perform other duties as assigned.

Job description

Purpose

The ACT Therapist provides community-based individual therapy, clinical, substance use disorder and support services to consumers and/or their support system utilizing the ACT Model. The ACT therapist will join the ACT multi-disciplinary team to serve those individuals with serious and persistent mental illness and presenting with the highest level of clinical intensity and at risk for arrest and/or higher levels of care.

Accountabilities

• Provide community based intensive therapy community-based services to adult consumers with a severe and persistent mental illness and may have a co-occurring substance use disorder who have frequent contact with criminal justice entities. These may also include other community-based services such as case management as needed
• Provides mental health, dual diagnosis and/or substance use disorder therapy, in both an individual and group setting, to consumers and families as appropriate
• Will maintain caseload of client/consumers from the ACT team and other teams as needed
• Assists with the development of diversion plans to help reduce/prevent hospitalizations and incarcerations; actively participate in team planning for implementation of diversion plans.
• Attend daily multi-disciplinary team meetings for coordination of care for clients.
• Actively participates in the research, development and implementation of emerging multi-system models and practices.
• Coordinates community / mobile emergency responses in conjunction with the Emergency & Stabilization Services team as needed.
• Participate in on-call or after-hours responsibilities when required by the program.
• Models inspire flexibility and creativity in the delivery of clinical services.
• Outreach individuals who are in need of mental health services and have had frequent law enforcement contact.
• Provide transportation and accompany clients to and from activities and/or appointments to provide medically necessary direct care and support, as directed.
• Complete all required documentation thoroughly, on time and according to agency standards.
• Attend team meetings and individual supervision sessions as required by the direct supervisor.
• Provides crisis prevention and intervention as needed.
• Perform other duties as assigned.
