Job summary

WellLife Network in New York is seeking an ACT Resource Manager to support programs by helping recipients obtain and maintain essential benefits. You’ll monitor eligibility, coordinate applications and recertifications, and work with ACT teams to ensure access to public and community resources.

The role involves collaboration with recipients, families, and agencies, and requires transporting clients, documenting in the EHR, and maintaining confidentiality.

Qualifications

• High school diploma or GED required; Bachelor’s in human services preferred. Minimum of 2 years’ experience working with individuals with mental illness.
• Two years experience working with Mental Health and Substance Abuse consumers preferred.
• Experience working with behavioral health population preferred.
• Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record.
• Valid New York State driver’s license and vehicle required – willingness to use own vehicle for home visits; regulatory background checks.

Responsibilities

• Monitor recipient eligibility and benefits across programs (Medicaid, Medicare, SNAP, SSI/SSDI, housing subsidies, etc.).
• Coordinate the full application and recertification process, including document collection and deadlines.
• Conduct regular eligibility reviews related to income, housing, treatment status, or other circumstances.
• Notify recipients and clinical teams of upcoming recertification deadlines and required documents.
• Collaborate with ACT clinicians to align benefits with treatment and recovery goals.
• Visit ACT sites and participate in team meetings to identify barriers to access and develop solutions.
• Transport and accompany recipients to DSS, HRA, SSA, and other agencies as needed.
• Educate recipients on benefit requirements, rights, responsibilities, and maintaining eligibility.
• Document contacts, updates, and services in the EHR within required timeframes.
