Job summary

Self Regional Healthcare Inc. in Missouri is seeking an Accountable Care Supervisor to lead the Accountable Care team within Population Health. This role coordinates staff activities, monitors productivity, and ensures timely completion of patient and practice-based workflows.

You will work closely with the Director of Population Health and other teams to advance quality, preventive, and value-based care initiatives, while focusing on improving patient outcomes and risk-adjustment efforts.

Qualifications

• Bachelor’s degree in healthcare administration, business, public health, nursing, or related field preferred.
• Relevant healthcare experience may be considered in lieu of degree requirements, asappropriate.
• Three years of healthcare experience preferred.
• Experience in physician practice operations, population health, quality improvement, or accountable care preferred.
• Previous supervisory or team-lead experience preferred.

The Accountable Care Supervisor provides day-to-day leadership and operational oversight for the Accountable Care team within Population Health. This position is responsible for coordinating staff activities, monitoring productivity and performance, supporting quality and risk-adjustment initiatives, and ensuring timely completion of patient and practice-based workflows.
The Supervisor works closely with the Director of Population Health and other Population Health teams to support organizational goals related to quality, preventive care, value-based care, and patient outcomes.
