Job summary

Piedmont Atlanta Hospital is seeking an Abdominal Transplant Clinical Pharmacist to deliver specialized pharmaceutical care for kidney, pancreas and liver transplant patients across all care phases. The role emphasizes multidisciplinary collaboration and optimizing transplant medication management.

The position rotates between kidney/pancreas and liver services in both inpatient and outpatient settings, with an opportunity to work from home one to two days per week depending on eligibility and

Qualifications

• Bachelor's degree from ACPE-accredited school of pharmacy or higher.

Responsibilities

• Provide direct patient care, including medication therapy management, education and counseling for transplant recipients and living donors.
• Participate in multidisciplinary inpatient rounds and contribute to clinical decision-making with the transplant team.
• Monitor and adjust immunosuppression, antimicrobial prophylaxis, and other transplant-related medications in addition to chronic disease state management.
• Conduct thorough medication reconciliation at transitions of care and assess for drug interactions, adverse effects, and adherence issues.
• Develop, implement, and monitor individualized pharmacotherapy plans for both inpatients and outpatients in accordance with institutional protocols and best practices.
• Provide transplant pharmacotherapy education to patients, caregivers, and healthcare professionals.
• Contribute to protocol development, quality improvement initiatives, and regulatory compliance for the transplant department.
• Participate in clinical research and data analysis to support optimal patient and program outcomes.
