BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for full-time or per diem opportunities throughout Montgomery County, Maryland.

Role Overview

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Responsibilities:

• Make home visits to clients in designated geographic areas.
• Perform diagnostic tests and measurements, including mobility, range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
• Continuously assess and update the care plan, collaborating with nursing staff as needed.
• Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using electronic devices.

Qualifications:

• Current Maryland Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records (EMR) preferred.
