Job summary

Provident Healthcare Consulting’s ELEVATE program seeks caregivers to provide compassionate, respectful support in home settings. You will ensure client health and safety, document all activities, and help with daily living tasks and personal care while maintaining confidentiality.

Training in CPI/Mandt and medication management will be required to start. The role involves supervising clients, reporting any abuse, accompanying to appointments, and performing duties on a 3rd shift schedule

Qualifications

• High school education or equivalent.
• Valid driver's license.
• Experience with individuals with mental illness or multiple conditions.
• Medication manager training preferred and training may be required.

Responsibilities

• Treat clients with dignity, respect and value.
• Ensure health, safety, and wellbeing of clients on shift.
• Report allegations of abuse to the Program Manager promptly.
• Complete required trainings and orientations within set periods.
• Document services, data sheets, incidents, and MAR during shifts.
• Support clients with personal goals and daily living activities.
• Assist with cooking, cleaning, laundry, meals planning and shopping.
• Help with daily living activities when needed.
• If certified, administer medications safely and document in MAR.
• Maintain confidentiality of client information.
• Implement behavior intervention plans when warranted.
• Report unsafe conditions or changes in client condition to Program Manager.
• Accompany clients to appointments and run errands as needed.
• Other duties as requested by Program Manager.
• N/B: CPI/Mandt and Medication Manager trainings will be required.
