Job summary

ELEVATE is seeking a Direct Support Professional for a 3rd shift weekend role in a 24/7 Intensive Residential Support Home. You will support clients with daily living, personal care, meals, social activities, and medication administration as needed, across all weekend shifts.

The role emphasizes dignity, safety, and privacy, with required trainings, documentation, and collaboration with the Program Manager to meet client goals. CPI/Mandt and Medication Manager training are anticipated.

Qualifications

• High school education or equivalent.
• Must have a valid drivers license.
• At least one year’s experience working with individuals with a mental illness or multi-occurring conditions.
• Medication manager or certified medication aide preferred (will be required to be trained in medication manager).
• Experience supporting individuals who experience problem behaviors preferred.

Responsibilities

• Treat all clients with dignity, respect, and value.
• Ensure health, safety, protective supervision, and wellbeing of clients on your shift.
• Immediately report any allegations of client abuse to the Program Manager.
• Complete all required trainings and orientations within the period specified by the Program Manager.
• Complete all required documentation on your shift before going home, including service documentation, data sheets, incident reports, and medication administration records.
• Provide support to clients with implementation of their personal goals and objectives.
• Provide supervision and assistance to clients with informal supports they may need help with, including cooking, cleaning, laundry, meal planning, shopping, and personal hygiene.
• Assist clients with activities of daily living/personal hygiene they are unable to do themselves.
• If certified to administer medications, ensure timely, safe administration and documentation in the MAR.
• Maintain confidentiality and privacy of client information.
• Implement client behavior intervention plans when needed.
• Report unsafe conditions in the home to the Program Manager.
• Report changes in client’s condition or family situation to the Program Manager.
• Accompany clients to scheduled appointments and transport them or run errands as needed.
• Perform other duties as requested by the Program Manager.

Position Summary:

The direct support professional is responsible for the daily support and oversight of clients living in the Intensive Residential Support Home. The DSP provides care for individuals living with disability and/or mental health illness by providing support and assisting with daily activities in their homes. This will include working on identified goals such as meal cooking and preparation, personal cares, promoting community involvement, social skills etc. The DSP will be responsible for administering medications during the shift needed. The home operates 24 hours a day. DSP’s will be scheduled over all shifts to ensure coverage.

Shift: 3rd Shift WEEKEND ONLY --- Satruday & Sunday 8pm-8am

Job Description

10. Responsible to maintain the confidentiality and privacy of client information.

11. Responsible to implement client behavior intervention plans when their problem behavior warrants.

12. Promptly report any unsafe conditions in the home to the Program Manager.

13. Reports changes in client’s condition or family situation to the Program Manager.

14. Accompanies clients to scheduled appointments and transports clients or running errands with or for clients.

15. Other reasonable duties as requested by the Program Manager.

Will need to secure CPI/Mandt Training, Medication Manager training
