Qualifications include a Master’s in Social Work and NYS LMSW or LHMC licensure, with at least one year in crisis management and two years in mental health. Bilingual skills are preferred to serve diverse communities.

Qualifications

• Master’s Degree in Social Work or related field from an accredited college or university.
• Must be licensed NYS LMSW or LHMC.
• Minimum of one year of crisis management experience in 24/7 settings.
• Two years of prior experience in the mental health field.

Responsibilities

• Provide integrated crisis response, crisis stabilization interventions and crisis prevention.
• Available 24/7, respond to crises and assess risk, mental status, and medical stability.
• Make referrals for emergency or continuing care and propose treatment alternatives.
• Develop or revise Crisis Plans in Person Centered Plans for new recipients.
• Coordinate movement across levels of care and discharge planning for community re-entry.
