Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Private Duty Licensed Practical Nurse to join our team in Unionville, TN. This on-site role involves delivering 1:1 patient care in a home setting with a fixed daytime schedule.

The ideal candidate has an LPN/LPT/LVN license, CPR certification, and 1+ year of nursing experience, preferably in home health. A valid driver’s license and insurance are required, with benefits including health coverage and PTO.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in the state of practice (TN).
• CPR certification is required; valid driver’s license and proof of auto insurance.
• 1+ years of nursing experience preferred; home health care experience preferred.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician
