Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Private Duty Licensed Practical Nurse to join our Spring Hill, TN team. You will provide 1:1 patient care in home settings, following each client’s plan of care with dignity and independence.

Requirements include an active LPN/LPT/LVN license, CPR certification, a valid driver’s license, and 1+ year nursing experience; home health experience preferred. Full-time daytime and overnight shifts available.

Qualifications

• Licensed Practical Nurse with active state license (LPN/LPT/LVN) required.
• Valid driver's license and proof of car insurance.
• CPR certification.
• 1+ year nursing experience; home health care experience preferred.

Responsibilities

• Providing 1:1 care for clients per plan of care while maintaining dignity and independence.
• Administering medications, feedings, oxygen, ostomy care and documenting actions and outcomes.
• Coordinating supplies, medications, and communication with family, case manager, and physicians.
• Ensuring all patient needs align with physician orders and care plans.
