Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Private Duty Licensed Practical Nurse to join our Germantown, TN team. This full-time Day shift position provides 1:1 patient care in the home, following each client’s plan of care with dignity and independence.

Responsibilities include administering medications, documenting nursing actions, coordinating supplies, and ensuring patient needs are met per physician orders.

Qualifications

• Valid LPN license in Tennessee.
• Home health care experience preferred.
• CPR certification required.

Responsibilities

• Providing 1:1 home care following the client’s plan of care.
• Administering medications, feedings, and other required treatments.
• Documenting nursing actions and communicating with family and team.
• Coordinating supplies and equipment to ensure on-hand availability.
• Ensuring patient needs are met per physician orders.
