Job summary

Community Partners is seeking an in-home personal care provider to work 1:1 with a child or adolescent with developmental disabilities and/or behavioral health challenges. The role supports goals from the individual’s In-Home Supports plan, focusing on increasing independence in daily living, social/communication skills, and life skills.

The ideal candidate has experience with disabilities, especially autism, is energetic, observant, and a clear communicator who can make community connections

Qualifications

• Experience working with individuals with disabilities, autism preferred.
• Must be a strong communicator and able to engage with families and youth.
• Must be able to drive; valid driver's license and proof of insurance required.
• Background check including criminal and driving records is required.

Responsibilities

• Provide 1:1 in-home personal care to children/adolescents.
• Work on goals from the In-Home Supports plan (daily living, social/communication, life skills).
• Make community connections on behalf of the child you support.
• Transport to appointments and activities as required.
