Job summary

Adoration Health is seeking a Part-time Home Health LPN to provide 1:1 patient care in the Elizabethtown, KY area. You will administer medications, perform basic nursing tasks, and document patient progress while collaborating with families and the care team.

Ideal candidates have an LPN/LPT/LVN license, CPR certification, and 1+ years of nursing experience, preferably in home health. This role offers flexible scheduling and stability with a growing organization.

Qualifications

• LPN/LPT/LVN Degree from an accredited nursing program with current licensure.
• Valid driver's license and proof of automobile insurance.
• CPR certification required for professional practice.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 home health care to patients per established plan of care.
• Administer medications, treatments and basic nursing procedures per physician orders.
• Document nursing actions and patient status; communicate with family and case manager.
• Coordinate supplies, medications, and equipment as needed to ensure on-hand availability.
• Ensure patient needs are met per physician instructions and plan of care.
