Job summary

BrightSpring Health Services’ Adoration Health unit seeks a qualified LPN for home health in Elizabethtown, KY. This part-time role covers Hardin, Grayson, Breckinridge and Meade Counties with 1:1 patient care and a focus on dignity and independence.

The LPN will administer medications and treatments per the plan of care, document actions, and coordinate with families and physicians. A valid LPN license and driver’s license are required.

Qualifications

• LPN/LPT/LVN degree with current state licensure
• CPR certification and valid driver's license
• Home health care experience preferred

Responsibilities

• Providing 1:1 home health nursing per plan of care
• Administering medications, oxygen, feedings, ostomy care as needed
• Documenting nursing actions and communicating with caregivers and physicians
• Coordinating supplies and ensuring patient needs are met
