Job summary

SSM Health in Waupun, WI is seeking a PA or NP for a full-time, 32-hour weekly occupational health role in an outpatient clinic. You will perform pre-employment and work-related physicals, develop population health programs, and collaborate with a dedicated MA and lab tech.

Qualifications

• Graduate of a nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) program.
• Ability to obtain PA/NP licensure and Wisconsin DEA license.
• APRN credential required for NPs.

Responsibilities

• Perform pre-employment and work-related physicals in outpatient clinic.
• Develop and contribute to population health programs for employer contracts.
• Provide patient-centered care with dedicated support staff and access to EMR (EPIC).
• Coordinate with labs, imaging, and specialty clinics onsite at SSM Health Waupun Memorial Hospital.
