Job summary

Genesis HealthCare System is seeking an RN Care Manager to act within a multidisciplinary team to ensure efficient patient progress and quality care. You will coordinate discharge planning for inpatients and some outpatients, addressing complex psychosocial needs and promoting wellness.

You will lead care planning with patients and families, participate in multidisciplinary rounds, and drive readmission reduction through coordinated transitions and post-discharge follow-up.

Qualifications

• Current Ohio RN licensure or LSW/LPC in Ohio.
• RN to have BSN with 3+ years or ADN with 7 years RN experience.
• LSW/LPC require Bachelor’s degree in Social Work.
• Ability to perform data analysis and use computer systems.
• Excellent verbal and organizational skills for case management.

Responsibilities

• Throughput & Coordination of Hospital Care with physicians and staff to improve outcomes.
• Collaborate with patient/family to set mutual goals and care plans.
• Facilitate multidisciplinary rounds and care coordination for assigned district.
• Assess 30-day readmission risks and develop resourceful discharge plans.
• Ensure CMS discharge checklist and maintain CMS/department standards.
• Promote patient safety and effective transitions of care and post-discharge follow-up.
• Perform pre-certifications for inpatient psychiatric admissions when needed.
• Coordinate home services and community resources after discharge.
