Job summary

Thye Hua Kwan Nursing Home Ltd is seeking a caring Staff Nurse to join our dedicated team. You will provide personalised nursing care to residents in line with established policies and standards, coordinating with doctors and multidisciplinary colleagues.

You will perform admissions assessments, administer medications safely, participate in care planning, and support residents’ wellbeing through compassionate, patient-centred care.

Qualifications

• Diploma or degree in nursing is required.
• Registered with the Singapore Nursing Board (SNB) with a valid practising certificate.
• Relevant nursing experience, ideally in eldercare, community hospital or nursing home settings.
• Compassionate, patient and committed to residents' wellbeing.
• Good communication skills and a team player.
• Willing to work rotating shifts, including weekends and public holidays.

Responsibilities

• Deliver skilled nursing care following approved standards of nursing practice.
• Carry out and supervise assessments of new admissions.
• Deliver care and nursing interventions as set out in each resident's care plan.
• Evaluate residents' responses and adjust the care plan as needed.
• Monitor residents' conditions and keep the team updated on changes.
• Prepare and administer medications safely; keep controlled drugs secure.
• Collaborate with doctors and multidisciplinary team to coordinate care.
• Initiate preventive and rehabilitative nursing procedures with Allied Health professionals.
• Prepare staff rosters and daily assignments for the ward.
• Participate in training and development of nursing staff.
• Handle emergencies promptly and escalate as needed.
• Maintain a safe, clean nursing home environment and participate in quality audits.
• Assist Nurse Manager with staff performance appraisals.

You will provide personalised, cost-effective care to our residents in line with established policies, procedures and nursing standards, working closely with doctors, allied health professionals and the wider care team.

• Deliver skilled nursing care that follows approved standards of nursing practice
• Carry out and supervise assessments of new admissions.
• Deliver care and nursing interventions as set out in each resident's care plan
• Evaluate how residents respond to care, and recommend changes to the care plan where needed
• Monitor residents' conditions and needs, and keep the care team updated on changes
• Prepare and give medications safely, and keep controlled drugs secure
• Work with residents' doctors and the multidisciplinary team to coordinate care
• Partner with Allied Health professionals to initiate preventive and rehabilitative nursing procedures
• Prepare staff rosters and daily assignments for the ward
• Take part in training and developing nursing and care staff
• Handle emergencies promptly, act quickly to contain the situation, and escape to your supervisor when needed
• Work with the team to keep the nursing home safe and clean
• Take part in quality assurance activities and regular quality audits
• Help the Nurse Manager with staff performance appraisals
• Take on other duties assigned by the Nurse Manager, Senior Nurse Manager or Head (Nursing)

Requirements
