ABER CARE PTE. LTD. in Singapore is seeking a Care Coordinator to create personalised care plans tailored to each resident's needs and to implement and monitor progress.

The role involves designing programs, coordinating reviews every six months, and collaborating with stakeholders to ensure comprehensive geriatric care. The ideal candidate holds a diploma/degree in Social Work, Psychology, or Counselling with at least 1 year of healthcare experience, strong interpersonal and problem-solving

Qualifications

• Min Diploma/Degree in Social Work/Psychology/Counselling
• At least 1 years relevant healthcare industry experience
• Good interpersonal and problem-solving skills
• Proficient in English and able to converse in dialects or other languages
• Office Hours: 5 days work week
• Enjoys in a Geriatric Care setting

Responsabilités

• Create personalised care plans tailor to the needs of resident
• implement care plans and regularly record residents' progress
• Design and organise programs and activities to address residents' care requirements
• Prepare and submit review reports and monthly interview records every 6 month
• Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents
