Job summary

ABER CARE PTE. LTD. Singapore is seeking a dedicated professional to create personalised care plans tailored to each resident's needs. You will implement these plans, monitor progress, and adjust as required to ensure high-quality care.

You will design and organise activities, prepare and submit review reports every six months, and collaborate with stakeholders to coordinate comprehensive care for residents. Monday to Friday shifts with DO rotation are expected.

Qualifications

• Diploma in Social Work or Psychology required.
• At least 2 years of relevant healthcare experience.
• Able to work Monday to Friday with shift DO.
