Job summary

Metta Welfare Association in Singapore seeks a Social Work Assistant to support Social Workers and the multidisciplinary team in enhancing residents’ well-being and coordinating programmes. You will provide administrative support and help with case management and community resources.

The role emphasizes compassionate, collaborative care for adults with disabilities in a residential setting, including documentation, subsidy processes, and engagement with families and service partners.

Responsibilities

• Conduct client assessments and manage admissions, care planning, reviews, and discharges.
• Monitor clients’ needs and coordinate care with the multidisciplinary team.
• Conduct home or hospital visits when required.
• Provide counselling, emotional support, and caregiver education.
• Assist with Means Testing, financial assistance applications, and access to community resources.
• Work closely with healthcare professionals and participate in case discussions and service planning to ensure holistic care.
• Maintain accurate client records, reports, and database updates.
• Prepare and submit subsidy claims, monthly reports, and required statistics.
• Ensure compliance with organizational policies and regulatory requirements.
• Supervise and guide Social Work Administration staff.
• Support audits, organizational projects, and events.
• Perform other duties assigned by management.
