Job summary

SINGHEALTH POLYCLINICS is seeking a Social Work Assistant to gather information for accurate assessments and to facilitate appropriate referrals. The role involves connecting patients with community resources and supporting a team addressing medical and psychosocial needs.

The SWA may accompany MSWs on home visits and assist with administrative tasks, meetings, outcomes tracking, and data collation to support financial aid and services.

Qualifications

• GCE O or A levels with related experience in social services or customer service.
• Diploma in Social Sciences or related field (Social Work/Psychology/Counselling/Community Care).
• Able to speak local languages.
• Proficient in Microsoft Office applications.
• Strong communication skills.

Job Description:

The Social Work Assistant (SWA) gathers corroborative information to support accurate assessments and facilitate appropriate service referrals. The SWA is to connect patients with suitable community resources and contribute to a team that provides comprehensive care addressing both medical and psychosocial needs. SWAs may accompany MSWs on home visits to gain deeper insights into patients' circumstances. Beyond direct patient interaction, the SWA is required to support administrative tasks such as organising meetings, tracking outcomes and data collation. The SWA’s work is essential in ensuring patients receive necessary financial aid and community support, while also contributing to the efficient operation and development of the Medical Social Services in SHP.

Requirements:

• GCE O or A levels with 6 years of experience in the social service sectors or customer service experience; or
• Diploma in Social Sciences (Social Work)/ Psychology Studies/ Counselling/ Community Care or Services/ Social Service/ Social Enterprise or its equivalent
• Able to speak local languages
• Proficient in Microsoft Office application
• Good communication skills
