Job summary

NHG Health is seeking a Staff Nurse to join our Community Health Team. You will work with healthcare professionals and community partners to support patients in their homes, promote independence and wellbeing, and contribute to better health outcomes in the community.

You will gain exposure to community nursing, care coordination and population health while building meaningful relationships with patients and families.

Qualifications

• Diploma or Degree in Nursing with a valid license in Singapore.
• Registered with the Singapore Nursing Board.
• At least 1 year of nursing experience.

Responsibilities

• Provide person-centred care and support to patients and residents in community settings.
• Conduct assessments and help develop care plans based on patient needs.
• Support patients and caregivers through education, coaching and guidance.
• Work with multidisciplinary teams and community partners to coordinate care and services.
• Facilitate smooth transitions of care across different healthcare settings.
• Monitor patients’ progress and escalate concerns when required.
• Participate in quality and patient safety initiatives.

You will help patients, residents and their caregivers navigate their healthcare journey, promote independence and wellbeing, and contribute to better health outcomes within the community. This role offers valuable exposure to community nursing, care coordination and population health, while allowing you to build meaningful relationships with patients and families.

Who We're Looking For

We are looking for a Registered Nurse with a Diploma or Degree in Nursing with a valid practising licence with the Singapore Nursing Board.

The ideal candidate is passionate about community nursing, with minimally one year experience in nursing.

You should possess strong communication and interpersonal skills, be adaptable and resourceful in a dynamic care environment, and have a strong willingness to learn and grow professionally.
