Job summary

Recruit Express Pte Ltd. is seeking a Social Worker / Senior Social Worker to support Community Care & Elderly Services across Singapore.

You will conduct assessments, develop care plans and provide psychosocial support for seniors and caregivers, with travel to different locations across the island. The role requires a degree in Social Work, SASW registration, and experience in case management, counselling or care coordination.

Qualifications

• Graduate Diploma or Bachelor's Degree in Social Work, SASW registered preferred.
• Registered Social Worker with the Singapore Association of Social Workers (SASW).
• Experience in case management, counselling, care coordination or community-based services advantageous.
• Willingness to travel across Singapore for client-related work.

Responsibilities

• Conduct assessments, care planning and interventions for seniors and caregivers.
• Develop, implement and review individualised care plans.
• Build rapport and provide psychosocial support to clients and caregivers.
• Advocate for clients' needs and coordinate appropriate community resources.
• Manage case referrals and maintain accurate case documentation.
• Participate in case conferences and multidisciplinary team (MDT) meetings.
• Monitor and evaluate coordinated services and case outcomes.
• Coordinate care services to support seniors to remain in the community.
• Provide information on relevant assistance schemes and care options.
• Support seniors and caregivers in advance care planning and end-of-life discussions.
• Collaborate with internal teams, healthcare professionals and external agencies.
• Conduct community outreach activities and support other ad-hoc initiatives where required.
