Job summary

Recruit Express Pte Ltd is seeking a Social Worker to join the Active Ageing Centre team in Singapore. You will support seniors to stay healthy, independent and connected within the community, collaborating with caregivers and professionals to identify needs early and provide psychosocial support.

You will conduct assessments, help develop integrated care plans, and drive community initiatives that promote resilience and social participation among seniors.

Qualifications

• Degree or Postgraduate Degree in Social Work recognised by SASW.
• Registered Social Worker preferred, or eligible for registration.
• Preferably at least 2 years of relevant Social Work experience in community care etc.
• Experience with seniors, caregivers or vulnerable populations would be advantageous.
• Community development, outreach, group work or community engagement experience would be an added advantage.

Responsibilities

• Engage seniors and caregivers to understand their needs, goals and strengths.
• Conduct psychosocial and financial assessments; develop integrated care plans.
• Provide counselling, emotional support and psychosocial interventions.
• Support seniors facing caregiver stress, grief, isolation, dementia, mental health challenges.
• Conduct outreach, screening and home visits as appropriate.
• Develop community initiatives to promote healthy ageing and connectedness.
• Connect seniors to activities, resources and appropriate care services.
• Work with healthcare professionals and partners to ensure continuity of care.
