Lions Home For The Elders in Singapore seeks a Physiotherapist to manage rehabilitation care for geriatric residents with physical and cognitive limitations. You will mentor Therapy Aides, adopt rehabilitation technologies, and develop programmes to improve resident outcomes.
You will lead clinical assessments, plan individual and group exercises, and collaborate with the multidisciplinary team to ensure evidence-based care. Registration with AHPC and 3+ years experience are required.
Manage the rehabilitation care of geriatric residents with medical, physical and cognitive limitations. The role also involves mentoring staff, adopting rehabilitation technologies and developing programmes to enhance resident care and outcomes.
Conduct comprehensive clinical assessments for new and existing residents and develop individualised rehabilitation care plans.
Plan group and individual exercises to maintain functional independence and prevent contractures or physical decline. E.g. fall prevention, functional mobility goals.
Conduct six-monthly reviews of rehabilitation programmes and make adjustments to optimise resident outcomes.
Ensure therapeutic interventions comply with organisational standards and clinical best practices.
Support the implementation of robotics and advanced healthcare technologies introduced by LHE.
Oversee the collection and analysis of rehabilitation programme feedback and data.
Guide and supervise Therapy Aides in delivering daily exercises, care plans and rehabilitation routines safely and effectively.
Lead clinical case discussions to strengthen the team's skills and knowledge
Support the Rehabilitation Manager in daily clinical operations and duty rostering.
Collaborate with the multidisciplinary team to develop integrated care plans.
Educate family members and caregivers on care plans and safe mobility techniques, where required.
Maintain accurate and timely clinical documentation in compliance with MOH, AHPC and organisational requirements.
Oversee the procurement, maintenance, safety checks and inventory of therapy equipment.
Participate in Continuous Quality Improvement (CQI) projects, clinical audits and service improvement initiatives.
Undertake regular professional and advanced skills upgrading to meet AHPC and Allied Health professional requirements.
Bachelor’s or Master’s Degree in Physiotherapy or a recognised equivalent qualification.
Full or Restricted Registration with the Allied Health Professions Council (AHPC), Singapore.
Minimum 3 years of relevant physiotherapy experience, preferably in geriatric, rehabilitation or community care settings.
Prior supervisory or leadership experience would be an advantage.
Excellent communication and interpersonal and problem-solving skills, with the ability to work effectively with residents, caregivers and multidisciplinary teams.
Able to manage multiple priorities in a fast-paced healthcare environment.
A good team player with initiative and experience in mentoring and coaching junior staff.
Perform any other duties as assigned by the Rehabilitation Manager or Management, in support of the Home’s operational and resident care needs.
