Job summary

active global respite care pte. ltd. in Singapore seeks a licensed physiotherapist to lead community rehabilitation services. You will assess needs, plan individualized care, and work within a multidisciplinary team to support elderly clients from admission through discharge.

You will supervise and train staff and students, maintain accurate records, and conduct home visits as needed. A 5‑day week with possible weekend rotation is expected.

Qualifications

• Diploma or Degree in Physiotherapy recognised by the Singapore Allied Health Professions Council.
• Must be a fully registered therapist with the Singapore Allied Health Professions Council.
• 5-day work week with rotational weekends may be required.
• At least 2 years of experience, preferably in community care.
• Hands-on experience and enjoys working with elderly in the community.
• Able to work independently with little supervision.
• Proficient in Microsoft Office and comfortable with IT.
• Able to communicate effectively with elderly clients.

Responsibilities

• Influence and shape a new model of Care: More Fun, More Engaging, More Effective with leading edge equipment and facilities.
• Oversee care of clients with direct responsibility for rehabilitation services as part of the management team.
• Assess, evaluate and identify rehabilitation needs of clients.
• Provide and advise necessary treatments.
• Formulate and implement individualized rehabilitation programmes to improve movement/function.
• Work with a multi-disciplinary team to create Individualised Care Plans from admission to discharge.
• Evaluate and monitor progress of clients’ treatment regularly.
• Ensure compliance with MOH regulations in service delivery.
• Guide and train therapists, therapy students, aides, family caregivers and volunteers.
• Create and maintain client database with accurate records.
• Conduct home visits when required.
• Communicate clearly with clients and colleagues, maintaining appropriate tone.
• Fulfill any other duties as assigned by Rehab Manager or Centre Manager.

• Be part of the management team in overseeing the care of clients with direct responsibility of rehabilitation services.
• Assess, evaluate and identify rehabilitation needs of clients.
• Providing and advising necessary treatments.
• Formulate and implement appropriate individualized rehabilitation programmes to facilitate improvement or maintenance of movement/function.
• Work closely in a multi-disciplinary team to create Individualised Care Plans for each client from admission to discharge planning.
• Evaluate and monitor progress of clients’ treatment regularly.
• Ensure strict compliance to MOH regulations in the performance of services.
• Guide, supervise and train conditionally registered physiotherapists, physiotherapy students, therapy aides, family caregivers and volunteers on their performance of duties.
• Create and maintain database of all clients, ensuring strict compliance to documentation, providing evidence-based treatment with accurate and timely records.
• Conduct home visits for clients when required.
• Communicate clearly and appropriately with clients & colleagues, always speaking with the understanding of tone and choice of words.
• Any other duties as and when assigned by Rehab Manager or Centre Manager.
