Job summary

WECRUIT PTE. LTD. is seeking a Pharmacist to oversee medication review, dispense medicines, counsel patients, and collaborate with doctors to ensure safe and effective therapy within our hospital setting.

You will supervise Pharmacy Assistants and Technicians, manage inventory, monitor compliance, and contribute to continuous quality improvement of pharmacy services.

Responsibilities

• Conduct clinical review of patients’ medication charts and prescriptions to assess suitability and safety.
• Identify adverse drug reactions, interactions and collaborate with doctors to resolve concerns.
• Oversee accurate processing and fulfilment of medication orders.
• Verify prescriptions, dispense medicines and provide patient counselling.
• Oversee medication-related discharge processes for inpatient and outpatient services.
• Provide drug information and professional support to doctors and healthcare professionals.
• Prepare extemporaneous pharmaceutical products when prescribed.
• Oversee pharmaceutical inventory management and stock replenishment.
• Supervise Pharmacy Assistants and Technicians to ensure effective operations.
• Liaise with external medical practitioners and hospital residents on medication matters.
• Monitor compliance with regulations governing receipt, storage, distribution and documentation of medicinal products.
• Ensure records are maintained for all stages of the medication supply process.
• Support continuous improvement of pharmacy processes, medication safety and service quality.
• Perform other responsibilities as assigned by the Senior Pharmacist or Pharmacy Manager.

Job description

• Conduct clinical review of patients’ medication charts and prescriptions to assess medication appropriateness, dosage, therapeutic limits and potential safety concerns.

• Identify potential adverse drug reactions, interactions and other medication-related issues, and collaborate with prescribing doctors to resolve identified concerns.

• Oversee the accurate and timely processing and fulfilment of medication orders.

• Verify prescriptions, dispense medicines and provide appropriate patient counselling in accordance with professional and institutional requirements.

• Oversee medication-related discharge processes for both inpatient and outpatient services.

• Provide drug information and professional medication support to doctors and other healthcare professionals.

• Prepare or oversee the preparation of extemporaneous pharmaceutical products when prescribed and where required.

• Oversee pharmaceutical inventory management, including stock records, ward stock checks and timely replenishment.

• Supervise, guide and support Pharmacy Assistants and Pharmacy Technicians to ensure effective pharmacy operations.

• Liaise with external medical practitioners and hospital-based Resident Medical Officers on medication-related matters when required.

• Monitor compliance with applicable requirements governing the receipt, storage, supply, distribution and documentation of medicinal products.

• Ensure appropriate records are maintained for all relevant stages of the medication supply process.

• Support continuous improvement of pharmacy processes, medication safety and service quality.

• Perform other responsibilities as assigned by the Senior Pharmacist or Pharmacy Manager.
