Job summary

WECRUIT PTE. LTD. is seeking a Clinical Pharmacist to provide expert medication management within our hospital team in Singapore. You will review charts, verify and dispense medications, and support clinicians with drug information and safety measures.

Qualifications

• Bachelor’s Degree in Pharmacy from an SPC-recognised university.
• Registered with the Singapore Pharmacy Council (SPC).
• Strong team player with effective communication and interpersonal skills.
• Ability to work effectively and independently with minimal supervision.

Responsibilities

• Review patient medication charts for appropriateness, dosage, interactions and adverse reactions.
• Accurately and promptly review, verify and dispense medications with counselling.
• Manage inpatient and outpatient medication discharges.
• Provide drug information and clinical support to doctors and staff.
• Prepare extemporaneous and compounded medications as prescribed.
• Maintain accurate medication inventory records and conduct stock checks.
• Supervise Pharmacy Assistants and Pharmacy Technicians.
• Liaise with external physicians and hospital Residents when required.
• Ensure all medication supply, storage and documentation comply with policies and regulations.
• Perform other duties as assigned by Senior Pharmacist or Pharmacy Manager.

Responsibilities for Clinical Pharmacist:

• Review patient medication charts and prescriptions for appropriateness, dosage accuracy, therapeutic limits, potential drug interactions and adverse reactions, and liaise with prescribers when intervention is required.
• Accurately and promptly review, verify and dispense medications, including providing appropriate medication counselling.
• Manage and oversee inpatient and outpatient medication discharges.
• Provide drug information and clinical support to doctors, medical staff and other healthcare professionals.
• Prepare extemporaneous and compounded medications as prescribed.
• Maintain accurate medication inventory records, conduct regular stock checks and ensure adequate ward stock levels through timely replenishment.
• Supervise and provide guidance to Pharmacy Assistants and Pharmacy Technicians.
• Liaise with external physicians and hospital-based Resident Medical Officers when required.
• Ensure all medication supply, storage, distribution and documentation comply with hospital policies and applicable legal and regulatory requirements.
• Perform other duties as assigned by the Senior Pharmacist or Pharmacy Manager.

Qualifications for Clinical Pharmacist:

• Bachelor’s Degree in Pharmacy from an accredited university recognised by the Singapore Pharmacy Council (SPC).
• Registered with the Singapore Pharmacy Council (SPC).
