Job summary

Agape Plus is seeking a Registered Nurse to serve as a Nurse Case Manager / Interim Caregiver Manager, providing clinical oversight, case management and supervision of caregivers delivering home-based care.

The role requires assessing clients' health and functional needs, creating tailored care plans and monitoring progress to ensure safe, patient‑centered home care across Singapore. It also involves coordinating caregiver deployment and performing regular reviews with families.

Qualifications

• Diploma or Degree in Nursing with SNB registration and Practising Certificate.
• 2+ years of clinical nursing experience preferred.
• Experience in home care, community or geriatric care is advantageous.
• Ability to assess care needs, develop care plans and escalate changes in condition.
• Experience supervising or coaching caregivers is preferred.
• Strong communication, interpersonal and case-management skills.
• Proficient documentation and report-writing.
• Comfortable using digital care-management systems and basic office apps.
• Willing to travel across Singapore for client home visits.
• Committed to client safety, dignity and confidentiality.

Responsibilities

• Assess clients' health and home environment to determine care needs.
• Review and develop individualised care plans; ensure safe delivery of care.
• Manage cases, conduct home visits and follow up with clients/families.
• Provide clinical oversight and supervision to caregivers and staff.
• Monitor condition changes and escalate to healthcare professionals as needed.
• Conduct client reviews and identify ongoing or longer-term care needs.
• Coordinate caregiver matching, deployment, scheduling and handovers.
• Communicate with clients, families, hospitals and care partners to support continuity of care.
• Participate in caregiver interviews, onboarding, coaching and training.
• Respond to incidents and concerns with proper documentation and follow-up.
• Maintain accurate care plans and incident reports; support QA and audits.
