Job summary

Institute of Mental Health (IMH) in Singapore invites applications for the role of Manager / Senior Manager (Community Connect). You will help develop and operationalise Community Connect, IMH's platform for partnerships, capability building and service integration across the community health ecosystem.

You will collaborate with clinical departments, community partners and health system stakeholders to strengthen care beyond hospital walls, drive system improvements, and build capabilities

Responsibilities

• Strategic planning and community mental health development: support planning and implementation of Community Connect priorities and workplans.
• Partnership development and stakeholder management: liaise with community partners and NHG Population Health.
• Capability building and ecosystem enablement: coordinate capability-building initiatives and training opportunities.
• Service integration and care transformation: facilitate integration of hospital and community-based services and new care pathways.
• Performance monitoring and operations: develop KPIs and prepare reports for Senior Management.
