The Institute of Mental Health (IMH) in Singapore seeks a Manager/Senior Manager for Community Connect to drive strategy, partnerships, and service integration across hospital and community settings.

You will work with clinical departments, community partners, and NHG stakeholders to strengthen care beyond hospital walls, improve transitions, and build ecosystem capabilities. Leadership and cross-functional collaboration are essential.

Responsabilidades

• Support strategic planning and implementation of Community Connect priorities.
• Conduct environmental scanning, population needs assessment and landscape reviews.
• Develop frameworks, proposals and business cases to support community mental health initiatives and ecosystem development.
• Serve as a liaison with community partners, agencies, NHG Population Health and other stakeholders.
• Coordinate partnership engagements and governance structures, escalation pathways and performance monitoring.
• Oversee coordination of capability-building initiatives for community partners and organise training.
• Support development of competency frameworks, standards, accreditation and quality assurance initiatives.
• Facilitate integration of hospital and community-based mental health services.
• Coordinate cross-functional projects involving clinical, operational and community stakeholders.
• Develop and track KPIs, prepare reports for Senior Management and governance platforms.
