Job summary

EnviroDynamics Solutions Pte Ltd is seeking a Clinical Pharmacist to join a leading public healthcare institution. The role focuses on reviewing prescribed medications, performing reconciliation, and providing drug information to healthcare teams.

You will participate in multidisciplinary rounds and ambulatory services, counsel patients, and supervise pharmacy staff while developing advanced clinical curricula and training objectives.

Qualifications

• At least 5 years of relevant experience as Clinical Pharmacist in an inpatient setting.
• PharmD or equivalent degree recognized by the Singapore Pharmacy Council.
• Experience in training and development of clinical curriculum and programs.
• Strong leadership and communication skills.

Responsibilities

• Review order forms to ensure prescribed medication is appropriate.
• Perform medication reconciliation and provide drug information to clinicians.
• Participate in clinical services, rounds, and ambulatory care; counsel patients on medications.
• Oversee and review the work of pharmacy assistants and technicians.

Job Responsibilities:

• Review written/electronic order form (medication chart, medication record or prescription) to ensure drug prescribed is appropriate.
• Perform medication reconciliation.
• Provide drug information service to health care professionals.
• Involvement in clinical services such as attending multidisciplinary rounds and running ambulatory services. Dispenses and counsel patient on medication prescribed by doctor.
• Assigns, oversees and closely review the work of pharmacy assistants and technicians.
• Training pharmacists to be advance generalists specialists. Craft out necessary learning objectives, curriculum, materials.

Requirements:

• At least 5 years' relevant experience as Clinical Pharmacist with inpatient setting experience
• PharmD or equivalent degree recognized by the Singapore Pharmacy Council
• Experience in training and development of clinical curriculum/training programs
• Strong leadership and communication skills
