Home Nursing Foundation seeks a dedicated Community Care Associate to support seniors in daily living, plan meaningful activities, and ensure a safe, engaging centre environment.

You will coordinate volunteers, run mobility and rehab activities, and respond to emergencies with CPR/AED. A WSQ Higher Certificate in Healthcare Support (Community Care) with at least 3 years' experience is required.

Qualifications

• Possess WSQ Higher Certificate in Healthcare Support (Community Care) with at least 3 years of work experience in a community care setting.
• Ability to support seniors in Activities of Daily Living and mobility activities.
• Experience coordinating volunteers and ensuring client safety and dignity.

Responsabilités

• Assist clients with ADLs including feeding, showering, toileting, and transfers.
• Plan and conduct activities for clients, including those with dementia.
• Perform ROM exercises and mobility training.
• Integrate maintenance rehab activities into daily routines for seniors.
• Monitor and record seniors' participation in activities.
• Manage and coordinate volunteer activities within the centre or externally.
• Plan and organize festival celebrations for clients.
• Escort clients between centre and destinations with or without transport.
• Perform CPR / AED and first aid in emergencies.
• Identify behaviours of concern and respond appropriately.
• Maintain cleanliness and safety, manage stock, and assist with admin tasks.
• Provide mentorship to new staff and support the care team.
